Chronic Disease Self-Management (CDSM) Program

Evidence-based health education and skill-building workshop series

The CDSMP Program is a six week evidence based Chronic Disease Self Management Program that equips individuals living with chronic illnesses with practical skills, tools, and support to improve their health outcomes and quality of life.

Participants will learn strategies to manage symptoms, reduce complications, and build long term healthy habits.

Six week sessions are held at various locations within Mecklenburg County

Registration is open to all.






Session 1

  • Living Well with Chronic Conditions
    -Introduction to chronic disease self-management
    -Understanding the physical, emotional, and social impact of chronic illness
    -Introduction to self-efficacy and personal responsibility
    -Goal setting and creation of first weekly action plan

Session 3

  • Healthy Movement & Nutrition
    -Safe physical activity for varying ability levels
    -Building movement into daily routines
    -Basic nutrition concepts for chronic disease management
    -Problem-solving barriers to healthy habits

Session 6

  • Maintaining Progress & Community Connection
    -Sustaining healthy behaviors long-term
    -Preventing setbacks and managing change
    -Identifying community and Library resources
    -Reflection, celebration of progress, and program completion

Session 2

  • Managing Symptoms & Stress
    -Pain, fatigue, and symptom management strategies
    -Relaxation techniques (breathing, guided imagery, gentle movement)
    -Identifying stress triggers and coping tools
    -Review and refinement of action plans

Session 4

  • Medications & Health Literacy
    -Medication management and adherence strategies
    -Understanding prescriptions and medical instructions
    -Navigating healthcare systems
    -Preparing questions and advocating during medical visits

Session 5

  • Communication & Emotional Well-Being
    -Effective communication with healthcare providers, family, and caregivers
    -Managing emotions such as frustration, anxiety, and depression
    -Building support networks
    -Strengthening confidence and self-advocacy
